Patient Information

To make an appointment and Consultation Procedure

You require a referral from your GP or another Specialist to see Dr Cochrane. A GP referral is valid for 12 months and a Specialist referral is valid for 3 months. Under the Health Insurance Act 1973 you must have a current referral when making appointments or you will not be eligible for the Medicare rebate.

You may ring and make an appointment  on  07 5531 3600. A first consultation letter and forms will be sent to you to complete and bring to your appointment. These forms ensure we have your correct information and data.

Please bring your referral, all scans and X-ray films or CD-ROMs, blood test results, relevant medical reports, a list of your current medications and anything you think may be relevant to make the best diagnosis.

When you come to your consultation you will give all scans and information to the staff at the front desk. At your consultation Dr Cochrane will read your information and look at your scans before your consultation. He will discuss your history, clinically examine you and discuss findings and treatment options with you.

Dr Cochrane will then document this discussion and send this information to your referring Doctor. If your referring Doctor is not your usual GP please inform Dr Cochrane or his staff so your GP can receive this information also.

If your symptoms change or you are finding it difficult to manage your pain please keep us informed. We do have a patient waiting list and if possible we will make an earlier appointment. We also recommend you revisit your GP. Your GP may wish to contact Dr Cochrane.

Dr Cochrane would encourage you to write any questions down and bring these to your appointment.
